    Transplant Program

    The Seattle Children’s Hospital (SCH) Transplant Center includes programs in heart, kidney, liver, and intestinal transplantation. SCH has the only Intestinal Rehabilitation Program, as well as the only intestinal transplant program, in the Pacific Northwest (PNW).

    Position Description

    Optimize medication therapies for solid organ transplant recipients, specifically: participate actively in family-centered rounds as the medication expert; prepare patient-specific immunosuppressive roadmaps for transplanted patients; ensure adherence to protocols; screen for drug-related problems and offer suggestions for optimal drug therapy to the transplant team; review drug levels and other lab values; make appropriate suggestions or adjustments to therapy; provide discharge teaching and coordination of medications to patients and families, provide information to staff concerning dosages, interactions, incompatibilities, adverse effects, administration, and appropriate use of medications. Your time in this position will be divided between decentralized inpatient clinical, ambulatory clinic and inpatient pharmacy responsibilities.



    Required Education/Experience:
    - Graduation from an accredited School of Pharmacy.
    - Hospital pharmacy experience.

    Required Credentials:
    - Licensed by the Washington State Board of Pharmacy as a pharmacist.

    - Pediatric hospital pharmacy experience and/or Pediatric Residency Training.
    - Solid Organ Transplant Residency Training
    - 1 year or more of Solid Organ Transplant pharmacy experience
