Monitor and process customer service requests for formula, medications, equipment, and/or supplies, supporting appropriate and timely patient care, treatment and delivery of services. Participate as part of clinical and operational teams to monitor patient compliance and use of materials to support safe, quality home care administration. Interface with care team partners and patients/caregivers to provide accurate and timely delivery of weekly, biweekly, or monthly shipments/ deliveries.

- One year experience in health care.

- AA degree or equivalent.
- One year experience in Home Care admissions planning and continuity of care issues including inventory management/insurance authorizations.
- One year experience in pediatric home care.
